Office of Student Life Display Case Reservation Form
*We do not supply tables, chairs, microphones, computers or projectors. These items must be reserved through Special Events.
Event/Items to be Displayed:
Note: Display cases cannot be locked. The Office of Student Life is not responsible for any lost or stolen items from the case.

This form must be submitted to the Office of Student Life at least 1 week prior to the start of display. |
| 2. |
Displays can only be in case(s) for 2 weeks unless approval is granted by the Director of Student Life. |
| 3. |
The display cases are reserved on a first come/first served basis to all registered student organizations. |
| 4. |
Displays not removed by the reserved group in a timely fashion will be removed by the Office of Student Life. |
| 5. |
Displays must adhere to the Posting Policy set forth in the Student Organization Handbook. |
Signatures from the organization's President and Advisor must be also be provided to the Office of Student Life.
