New Employee Information Form

Only the department supervisor, chairperson, or dean may submit this form to request a USC Upstate network ID account for new faculty or staff. This form is not to be used for students. Please provide the requested information as accurately as possible.

In addition to submitting this form to request a USC Upstate network ID account, the new employee must also claim their USC Columbia network ID account. The employee will soon receive an automated email from USC Columbia with instructions to claim their USC Columbia network ID account. This email will be sent to the employee’s personal email address listed in their employment application. Please inform the employee to check their junk folder.

Note that the USC Upstate account creation process is not complete until the new employee claims their USC Columbia network account. This entire process may take several business days. Once the USC Upstate account is created, the Technology Infrastructure and Operations Department will send, via e-mail, the account information to the requester.

All new employees must claim their USC Columbia network account before their new USC Upstate account is created. Submission of only this form without claiming the USC Columbia network account will result in a delay or cancellation of the entire account creation process.
