Change Forms

  • Change of Address 
    No form required. Please update on SSC and verify you have no registration holds.

  • Change of Catalog Year Form
    Use this form to change your catalog year for your degree program.

  • Change of Name Form 
    Change of Name Form (Online)
    Complete the form and attach legal documentation (marriage certificate, divorce degree, adoption papers) and return to the records office.

Application for Graduation

Submit application online through Self Service.
