All employees of the University of South Carolina Upstate who are paid through the University payroll system are covered by Workers' Compensation, if they are injured while working. In addition, student internships and practicums sponsored by various departments, including the Mary Black School of Nursing, off-campus and non-paid work experience coordinated through Career Services, are also covered. Any accidental injury or occupational disease sustained by employees in the course of performing their job duties may be covered.
When an Injury Occurs
It is important that when you are injured on the job, you immediately tell your supervisor so that medical treatment may be authorized and a Workers' Compensation claim filed. If the injury is not reported immediately, you may be denied the right to compensation. Refer to the Workers' Compensation Policy for additional details, including time lost from work and required forms.
For all employees, three important steps must be followed to ensure compensation:
For emergency injuries: Dial 911 or go to the nearest Emergency Room. Then, contact your supervisor and CompEndium Services at 877-709-2667. CompEndium is available 24/7.
For non-emergency injuries: Together, the supervisor and injured worker call CompEndium Services immediately at 877-709-2667.
CompEndium assists you in processing and scheduling your work related injury for treatment at an authorized treatment facility and claims handling with your insurance provider.
The injured employee and his/her supervisor complete the USC Employee and Supervisor Reports. Fax the completed forms, along with the Physician's Report , to CompEndium Services at 877-710-2667. If the doctor states you must miss work due to the injury, complete a Workers' Compensation Benefits Election form, located at the bottom of the Employee Injury Report, stating whether you want to use annual leave and/or sick leave, or receive Workers' Compensation.
All such injuries MUST be reported to the supervising faculty member and the charge person in the affiliated facility for investigation and identification of the source, if known. (If the exposure occurs in a health care facility, both the charge nurse & infection preventionist are to be notified for further investigation & testing of the source patient.) All follow-up visits for this type of injury are to occur at the Occupational Health Center in Spartanburg.
