Non-Scheduled Course form

Change of Address
No form required. Please update on SSC and verify you have no registration holds.

Change of Catalog Year Form 
Use this form to change your catalog year for your degree program.

Change of Name Form 
Complete the form and attach legal documentation (marriage certificate, divorce degree, adoption papers) and return to the records office.

Change of Social Security Number for Academic Record Form 
Complete form and return with a copy of your new social security card.

Change of Major Form 
Complete, obtain required signatures(s) and return to records.

Declaring / Deleting a Minor Form 
Complete, obtain required signatures(s) and return to records.

Transient / Concurrent Permission Form 
Complete, obtain required signatures(s) and return to records. Take a copy of the signed form to the school you are attending. Remember to send an official transcript to the records office after you complete the course. It will then be posted on SSC.

Pass/Fail Option Form
Complete, obtain required signatures(s) and return to records.

Change of Campus - Columbia (to USC Columbia)
Follow this link to complete the form online.

Change of Campus (all other campuses)
Complete the change of campus form in Self Service Carolina.

Grade Forgiveness Policy Handout and Grade Forgiveness Application 
Read the policy completely and if you qualify, please fill out and return the application.

Course Overload Form
Complete, obtain required signatures(s) and return to your academic unit for processing.

Suspension Appeal Form
Complete and return prior to the start of the semester you are hoping to attend.

Request to Change to Audit Form
Complete, obtain required signatures(s) and return to records.

Credit by Exam Form
After receiving permission from the Dean/Department Head to take the test, go to Enrollment Services to pay the exam fee and follow the remaining steps listed on the form to complete this process.

Request to have WF’s changed to W’s Form
Complete, obtain required signatures(s) and return to records.

Petition to Waive Academic Residency (the last 25%) and/or Course Substitution
Complete, obtain required signatures(s) and return to records.

Academic Grievance Appeal Form
Read the procedure carefully and if you decide to proceed use this form.

Reinstatement Form
To be used after second cancelation classes have started and late registration is over.
